How does race in American culture work with other factors to produce poorer health overall, i.e., not just for “African Americans”?

Some say we live in a “postracial” society. How does race in American culture work with other factors to produce poorer health overall, i.e., not just for “African Americans”?
And should make a compelling argument either that race has become irrelevant and other factors compromise Americans’ health, or that race is still important (at least in conjunction with other factors) in determining how healthy Americans can be today.
