1) List one target behavior you are contemplating making changes too. Use the S.M.A.R.T. model to make sure it is an appropriate behavior change for this reflection. (Specific, Measurable, Achievable, Relevant, Time-Based)
2) Paragraph two: Predisposing Factors. What knowledge do you have in regards to this target behavior? What knowledge will you need to acquire prior to making your behavior change? You may discuss your attitudes, beliefs, values or perceptions at this time. On a scale of 1(being very low) to 10 (being very high) where do you perceive your risk if you do NOT make this target behavior change? On a scale of 1(being very low) to 10 (being very high) where do you place the health benefits on the scale if you do decide to make the change?
3) Paragraph three: Enabling Factors: What skills, resources, facilities, or physical or mental capacities do you have access to or bring to the equation in order to be successful in making this behavior change?
4) Paragraph four: Reinforcing Factors. Do you have a social support network that can aid you in successfully making this behavior change? Discuss praise, possible rewards, encouragement and recognition. Now would be a good time to discuss Self Efficacy, Locus of Control and the dynamics between internal and external factors.
